LNF Application and project reporting forms
Applying for the grant:

After you have received a grant:

Best practice case study form. Please submit this form as soon as you receive your grant. (.pdf)
Interim report form (also known as the 6 month report form). Some groups will be required to complete an interim report form. If you are required to do so, it will have been included in the letter that you received with your cheque. (.pdf)
Change of use request. All changes to grant use require approval by the grant officers prior to changes taking place. (.pdf)
End of grant report form. The end of grant report form is due within 12 months of when you receive your cheque. Please contact us for a copy of this form.
Second funding request form. If you have already received LNF funding but are applying for a second time, you will need to complete this form prior to assessment. (.pdf)
Sample monitoring forms. These forms will be completed by case officers and/or panel members if your project is visited. (.pdf)
